APPLYING FOR AN ARKANSAS PATIENT REGISTRY ID CARD
Step 2:
Obtain a Physician Written Certification. Be prepared to start the application process with your physician certification, Arkansas-issued ID and payment method for required fees.
Step 3:
Create an AMMSys account to apply online or
download the application form and mail it in.
Note that online applications are preferred. A
valid email address is required for online applications.
